Leave request form guidelines

Please follow the guidelines below to properly complete the leave request form. It is important to provide accurate and detailed information to ensure smooth processing of your leave application.

• Employee information
  • Fill in your first name, last name and emailaddress
• Leave details
  • Indicate the duration of your leave by specifying the start and end dates
• Leave balance
  • Check your leave balance on your payslip. If you do not have sufficient leave days, unpaid leave applies
• If you have housing through Feel Flex
  • Please indicate whether you want to keep your room.
